a) As of the 7th of April 2017 is the nominating organisation (refer question 1) a (fully paid) current member of the Water Industry Alliance? *

b) Has the nominating organisation (refer to question 1) had a significant role in the delivery and or development of the nominated project/initiative? *

c) Did the nominated initiative start, or been further developed in the last 12 months? *

d) Do you agree that any information provided as part of this application (this form and any attachments) can be used for promotional purposes by the WIA? *

5. Please provide the name of representative attending Awards Ceremony to accept the Award, if successful? *Please note, winners will not be notified prior to Awards Ceremony *

6. Company Profile. Please provide an 80 word company profile that will be used in the Awards Program and any associated promotion. *

7. Smart Water Award Categories

Members may choose to nominate for more than one award category for a project/initiative, or alternatively submit more than one project/initiative.

However, each nomination MUST be submitted on a separate form.

8. Additional Award - Minister's Award for Excellence

In ADDITION to the above categories members may ALSO nominate the project/initiative for the Minister's Award for Excellence.
